bestemte forskningsmetodologier, som vil v\u00e6re relevante for dit forskningsprojekt. Du vil l\u00e6re, hvordan man udf\u00f8rer forskellige test og unders\u00f8gelser (f.eks. inklusion af patienter, blodpr\u00f8vetagning, lungefunktionsunders\u00f8gelser, endotel dysfunktionstest osv.) alt efter hvilket projekt, du arbejder med. fondsans\u00f8gning for f\u00f8rste gang. Jeg lavede specifikt en ans\u00f8gning til Kr\u00e6ftens Bek\u00e6mpelses skolarstipendiat. Jeg var b\u00e5de meget sp\u00e6ndt og usikker p\u00e5 samme tid, idet jeg aldrig havde arbejdet p\u00e5 en fondsans\u00f8gning f\u00f8r. Jeg var is\u00e6r usikker p\u00e5, hvordan en fondsans\u00f8gning skulle struktureres, hvad den skulle indeholde, og hvordan projektet skulle pr\u00e6senteres i ans\u00f8gningen.  P\u00e5 trods usikkerheder, blev min fondsans\u00f8gning im\u00f8dekommet \u2013 takket v\u00e6re exceptionel vejledning og motivation fra CSS-teamet.<\/p>\n<p>Til et af mine vejlederm\u00f8der f\u00f8r min ans\u00e6ttelse p\u00e5 CSS, havde jeg et m\u00f8de med min prim\u00e6re vejleder Ismail G\u00f6genur og min sekund\u00e6re vejleder Malene Broholm Andersen, hvor vi lagde en f\u00e6lles plan for min ans\u00e6ttelse og min tid som forsknings\u00e5rsstuderende, herunder plan for fondsans\u00f8gninger. Kort efter dette m\u00f8de, introducerede og viderehenviste Ismail mig til forskningskoordinatoren p\u00e5 CSS, Tove Kirkegaard Clausen, der foreslog, at vi bl.a. kunne sende en fonsans\u00f8gning til Kr\u00e6ftens Bek\u00e6mpelse. Til at starte med var jeg meget usikker og t\u00f8vende omkring dette, da jeg slet ikke kunne forestille mig, at jeg skulle f\u00e5 bevilliget min allerf\u00f8rste fondsans\u00f8gning. Men Tove motiverede mig, hvorefter hun guidede mig igennem de formelle dele af fondsans\u00f8gningen. Mens jeg arbejdede med ans\u00f8gningen, stod b\u00e5de Tove, Ismail og Malene konstant til r\u00e5dighed med v\u00e6rdifulde fif og feedback. Efter jeg f\u00e6rdiggjorde ans\u00f8gningen, krydsede vi alle fingre og sendte den afsted. \nNogle m\u00e5neder senere kom den gl\u00e6delige nyhed, at jeg fik bevilliget 120.000 kr. fra Kr\u00e6ftens Bek\u00e6mpelse. Jeg var meget overrasket, da det var meget uforventet for mig. Jeg havde ingen forventning om at blive bevilliget noget ved f\u00f8rste fors\u00f8g, og s\u00e5 i stedet arbejdet med ans\u00f8gningen som en l\u00e6reringsproces, hvor jeg ville f\u00e5 indsigt i, hvordan fondsans\u00f8gninger skulle laves. Forst\u00e5elsen for hvordan klinisk evidens bliver genereret har v\u00e6ret uundv\u00e6rlig i de sidste \u00e5r af l\u00e6gestudiet og det viderer virke som l\u00e6ge for mig. En ans\u00e6ttelse som scholar hos CSS giver desuden en fantastisk mulighed for bygge et netv\u00e6rk. I l\u00f8bet af mit forsknings\u00e5r lagde jeg fundamentet til tre originalartikler omhandlende ventralhernier, hvoraf den ene <a href=\"https:\/\/jamanetwork.com\/journals\/jama\/fullarticle\/2600464\" target=\"_blank\" rel=\"noopener\">blev antaget i JAMA<\/a>. Yderligere var jeg medforfatter p\u00e5 to artikler. Jeg oplevede, at der i Center for Surgical Science blev skabt rigtig gode rammer for, at jeg kunne udvikle mig som forsker. Da jeg startede havde jeg ingen forskningserfaring, men en stor interesse indenfor kirurgi. Mine vejledere var dedikerede og t\u00e5lmodige og var meget opm\u00e6rksomme p\u00e5, at jeg blev krediteret for alt det arbejde jeg lavede. Udover mine vejledere var jeg tilknyttet resten af forskningsenheden, hvor der var andre skolarer og ogs\u00e5 mere seniore forskere. Jeg delte kontor med to PhD studerende og en skolar. Det skabte et rigtig godt milj\u00f8 for faglig sparing, og der var altid nogen man kunne g\u00e5 til, hvis man ramte ind i en udfordring, ogs\u00e5 selvom vi alle forskede indenfor forskellige felter. Sammenholdet i enheden var stort og hver fredag spiste vi morgenmad med undervisning i diverse relevante emner. Udover at skrive artikler, var jeg i forbindelse med mit forsknings\u00e5r til flere nationale og internationale kongresser, hvor jeg pr\u00e6senterede min forskning. Blandt andet deltog jeg i American College of Surgeons \u00e5rlige kongres to \u00e5r i tr\u00e6k, hvor jeg pr\u00e6senterede mine studier med mundtlige pr\u00e6sentationer. Dette har gjort at jeg har f\u00e5et et stort netv\u00e6rk, og uanset hvilken kirurgisk afdeling jeg sidenhen har v\u00e6ret ansat p\u00e5, har jeg altid m\u00f8dt nogen kendte ansigter.<\/p>\n<p>De tre originalartikler er sidenhen sammen med en artikel mere blevet til min PhD afhandling. Jeg har siden min ans\u00e6ttelse som forsknings\u00e5rsstuderende v\u00e6ret tilknyttet Center for Surgical Science uafh\u00e6ngigt af, hvor min kliniske hverdag har v\u00e6ret. Som ung l\u00e6ge har vi meget skiftende arbejdspladser, og der har det v\u00e6ret rigtig dejligt at have et stort netv\u00e6rk og en faglig base at vende tilbage til. Flere af de forskere jeg arbejde sammen med i Center for Surgical Science er udover rigtig gode kollegaer sidenhen blevet mine gode venner. Jeg er stadig forskningsaktiv hos Center for Surgical Science samtidig med jeg er i gang med min hoveduddannelse i kirurgi og i det omfang det passer ind i mit familieliv. Vi planlagte hurtigt, hvad mit forsknings\u00e5r ville indeb\u00e6re; jeg skulle skrive et systematisk review og tage del i dataindsamlingen p\u00e5 et klinisk studie. De f\u00f8rste uger gik med at l\u00e6se og l\u00e6re om emnet, som jeg skulle skrive om, samt l\u00e6re de praktiske f\u00e6rdigheder forbundet med den daglige drift af et klinisk fors\u00f8g. Herefter skrev jeg, i t\u00e6t samarbejde med mine vejledere, en detaljeret protokol til det systematiske review.<\/p>\n<p>\u00a0\u00a0\u00a0\u00a0\u00a0 I juni m\u00e5ned rejste hele enheden til Tyrkiet p\u00e5 forskningsretreat. Programmet var omfattende og indeholdt artikelskrivning, genneml\u00e6sning, tr\u00e6ning i pr\u00e6sentationsteknik og meget mere. Inden afgang modtog vi undervisning i, hvordan man kan diktere sin forskning. Dikteringsmetoden har mange fordele, og is\u00e6r medf\u00f8rer den, at selve skriveperioden, som kan forekomme uoverskuelig, reduceres til at vare meget f\u00e5 dage. P\u00e5 retreatet modtog og gav vi feedback p\u00e5 hinandens arbejde, og mange endte derfor med at forlade Tyrkiet med n\u00e6sten f\u00e6rdige artikler. Det kr\u00e6ver grundig forberedelse og en gennemt\u00e6nkt artikelstruktur at anvende dikteringsmetoden, men den kan ende med at udg\u00f8re et essentielt v\u00e6rkt\u00f8j i forskningen. At till\u00e6re sig metoden tidligt er at g\u00f8re sit fremtidige jeg en tjeneste.<\/p>\n<p>\u00a0\u00a0\u00a0\u00a0\u00a0 Den n\u00e6ste halvdel af mit forsknings\u00e5r gik med at f\u00e6rdigg\u00f8re og publicere <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31892194\/\" target=\"_blank\" rel=\"noopener\">det systematiske review<\/a>, fungere som medforfatter p\u00e5 et par andre artikler samt at samle data p\u00e5 det kliniske fors\u00f8g. Sidstn\u00e6vnte udgjorde en mulighed for at begynde de artikler, som jeg arbejder p\u00e5 i dag. I PUMAS unders\u00f8ger vi effekten af fjern-isk\u00e6misk pr\u00e6konditionering hos patienter, der f\u00e5r foretaget en laparoskopisk kolecystektomi p\u00e5 grund af akut kolecystit.  S\u00e5 den ene gruppe f\u00e5r foretaget fjern-isk\u00e6misk pr\u00e6konditionering inden deres operation, og den anden gruppe er kontroller.<\/p>\n<p>Som forsknings\u00e5rsstuderende hos CSS, har jeg f\u00e5et nye erfaringer og stor rutine inden for f.eks.<\/p>\n<ul>\n<li style=\"text-align: center;\">Patientkommunikation<\/li>\n<li style=\"text-align: center;\">Blodpr\u00f8vetagning<\/li>\n<li style=\"text-align: center;\">Anvendelse af udstyr man normalt ikke st\u00f8der p\u00e5 i en klinisk hverdag<\/li>\n<li style=\"text-align: center;\">At f\u00e6rdes i laboratoriet og bruge laboratorieudstyr<\/li>\n<li style=\"text-align: center;\">At arbejde med eget projekt og skrive videnskabelige artikler<\/li>\n<li style=\"text-align: center;\">At skrive fondsans\u00f8gninger<\/li>\n<\/ul>\n<p><strong>En typisk dag med patientinklusion<\/strong><\/p>\n<p>Jeg starter min morgen med at tjekke operationsprogrammets, de kirurgiske sengeafdelingers og akutmodtagelsens patienter for at se, om der er nogle patienter, der er kandidater til at indg\u00e5 i studiet. Hvis der er en patient, der kan indg\u00e5 i studiet, g\u00f8r jeg min vogn klar med det m\u00e5leudstyr, de dokumenter og det blodpr\u00f8vetagningsudstyr, jeg skal bruge til inklusionen, og s\u00e5 g\u00e5r jeg op til patienten. Hvis patienten har lyst og overskud, fort\u00e6ller jeg om fors\u00f8get, og hvad der skal laves af unders\u00f8gelser og blodpr\u00f8ver i forl\u00f8bet. En stor del af inklusionen g\u00e5r ud p\u00e5 at danne en god patientrelation, s\u00e5 patienterne f\u00f8ler, de er i gode h\u00e6nder og kan v\u00e6re trygge ved at deltage.<\/p>\n<p>Patienten skal have tid til at l\u00e6se udleveret materiale om fors\u00f8get i fred og ro, inden patienten beslutter sig, s\u00e5 i mellemtiden g\u00e5r jeg ned og g\u00f8r laboratoriet klar til, hvis patienten gerne vil v\u00e6re med.<\/p>\n<p>Hvis patienten giver sit samtykke til at v\u00e6re med, tager jeg s\u00e5 en r\u00e6kke baseline blodpr\u00f8ver og m\u00e5linger p\u00e5 patienten og udfylder et sp\u00f8rgeskema med patienten. Derefter g\u00e5r jeg ned i laboratoriet og forarbejder blodpr\u00f8verne. I PUMAS tager vi mellem fem og seks forskellige blodpr\u00f8ver mellem fire og fem gange per patientforl\u00f8b. Blodpr\u00f8verne skal alle sammen behandles forskelligt i laboratoriet.<\/p>\n<p>Hvis patienten skal have lavet fjern-isk\u00e6misk pr\u00e6konditionering, g\u00f8r jeg s\u00e5 det som det n\u00e6ste - s\u00e5 t\u00e6t p\u00e5 deres operationstid som muligt. Herefter skal de igen have taget blodpr\u00f8ver, som skal forarbejdes i laboratoriet.<\/p>\n<p>N\u00e5r patienten er blevet opereret, laver jeg de forskellige m\u00e5linger og blodpr\u00f8ver igen henholdsvis to timer, fire timer og 24 timer efter deres operation. Derudover ringer jeg til mine inkluderede patienter 30 dage efter deres operation for at h\u00f8re, hvordan de har det, om de har v\u00e6re genindlagt og for at udfylde et sp\u00f8rgeskema med dem.<\/p>\n<p><strong>En typisk dag uden patientinklusion<\/strong><\/p>\n<p>Som forsknings\u00e5rsstuderende hos CSS f\u00e5r man sit eget projekt, man kan arbejde med og udgive en artikel om. Det arbejder jeg med, n\u00e5r jeg ikke har patientinklusion. Jeg har ogs\u00e5 brugt tiden til at skrive en del fondsans\u00f8gninger til mit eget projekt.<\/p>\n<p>Fordi jeg arbejder med akutte patienter, som man ikke kan forudse hvorn\u00e5r kommer, og som kan blive opereret b\u00e5de morgen og aften, s\u00e5 har jeg nogle gange lange dage med patientinklusion. I begyndelsen skulle jeg kun hj\u00e6lpe en Ph.d.-studerende med at inkludere patienter til hans studie og n\u00e5r jeg ikke hjalp til, havde jeg tid til at udf\u00f8re et systematisk review. Mine vejledere fortalte mig s\u00e5, at de havde ans\u00f8gt om tilladelser til at k\u00f8re en ekstra unders\u00f8gelse som et substudie. Da tilladelser s\u00e5 kom, blev jeg ansvarlig for at k\u00f8re et mindre randomiseret klinisk fors\u00f8g, som s\u00e5 blev mit eget projekt. Det var sv\u00e6rt is\u00e6r fordi jeg endnu ikke havde haft nogen klinisk erfaring. Det var stressende og udmattende kun at have et halvt \u00e5r til f\u00e5 inkluderet alle sine patienter. Det var h\u00e5rdt arbejde men jeg fik det klaret og i den sidste uge af mit forsknings\u00e5r fik jeg afsluttet den sidste patient i mit studie. Jeg havde en masse r\u00e5data da jeg startede p\u00e5 universitetet igen. Jeg gennemf\u00f8rte alle analyserne i min fritid. Jeg skrev min <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/31355944\/\" target=\"_blank\" rel=\"noopener\">artikel<\/a> et \u00e5r efter jeg havde f\u00e6rdiggjort min forsknings\u00e5r og det tog yderligere et \u00e5r at f\u00e5 det udgivet. I begge forl\u00f8b har jeg haft Ismail G\u00f6genur som vejleder.<\/p>\n<p>Mit forsknings\u00e5r blev enormt l\u00e6rerigt og givende for mit fremtidige virke som l\u00e6ge og forsker. Ismail er en vejleder, der er meget optaget af at uddanne sine ansatte til at de blive dygtige, selvst\u00e6ndige forskere. Ved starten af mit forsknings\u00e5r blev der hurtigt lavet en plan og en retning for \u00e5ret, hvor det samtidig blev forventet at jeg selv kunne tage ansvar. Dog under trygge forhold med kyndig vejledning.<\/p>\n<p>Allerede under min ans\u00e6ttelse som forsknings\u00e5rsstuderende havde vi mange sp\u00e6ndende videnskabelige diskussioner og Ismail gav frihed til at komme med egne ideer og m\u00e5l for fremtidige forskningsprojekter. Dette var med til at danne fundamentet til min ph.d. ans\u00e6ttelse. Under t\u00e6t vejledning med Ismail fik jeg defineret mit ph.d. forl\u00f8b og havde protokoller klar til kliniske fors\u00f8g inden jeg afsluttede mit forsknings\u00e5r.<\/p>\n<p>Jeg har nu haft Ismail som vejleder og mentor siden 2014.
